Migraine During Perimenopause: Why Hormones Matter More Than You Think

For many women, perimenopause brings far more than hot flashes and irregular periods.

One of the most frustrating symptoms I discuss with patients at Wellness MD is migraine. Some women who have never experienced migraine suddenly begin suffering debilitating headaches in their 40s. Others who have lived with migraine for years notice that attacks become more frequent, more severe, and far less predictable.

A new 2026 study published in The Journal of Headache and Pain helps explain why.

What did the researchers study?

El-Ghoneimy and colleagues examined 66 women between the ages of 40 and 55, comparing:

  • women whose migraine began during perimenopause,
  • women who had migraine before entering perimenopause, and
  • healthy women without migraine.

Each participant completed validated headache questionnaires while researchers measured estradiol (estrogen) and FSH levels.

Although relatively small, the study provides valuable insight into how hormonal changes influence migraine during the menopausal transition.

What did they find?

Women whose migraine started during perimenopause typically experienced:

  • moderate to severe throbbing headaches
  • pain around the eye, forehead and temple
  • frequent attacks
  • marked sensitivity to light and sound
  • poor response to pain medication
  • frequent analgesic overuse

Women who already had migraine before perimenopause often experienced even greater disability, including more menstrual-related migraine, prolonged symptoms after attacks, and poorer response to treatment.

Perhaps equally important, both migraine groups reported significantly more:

  • fatigue
  • sleep disturbance
  • anxiety
  • mood changes

This reminds us that migraine during perimenopause rarely occurs in isolation. It is often part of a much broader hormonal transition affecting multiple body systems.

The estrogen connection

One of the most important findings was that women with migraine had significantly lower estradiol levels than healthy controls.

However, the story is more complex than simply having “low estrogen.”

Current research strongly supports what is known as the estrogen withdrawal hypothesis.

Rather than absolute estrogen levels causing migraine, it is the rapid rise and fall of estrogen that appears to trigger attacks.

During reproductive life, estrogen follows a relatively predictable monthly rhythm.

During perimenopause, that rhythm disappears.

Some cycles produce very high estrogen levels. Others produce very little. Ovulation becomes inconsistent, progesterone production falls, and hormone fluctuations become increasingly unpredictable.

For the brain, these hormonal swings can be particularly challenging.

The science behind migraine

Estrogen influences one of the most important migraine pathways involving calcitonin gene-related peptide (CGRP).

CGRP is a powerful neuropeptide involved in pain transmission and inflammation within the trigeminovascular system, the network responsible for migraine.

Research has shown that declining estrogen increases CGRP activity, making the nervous system more susceptible to migraine attacks.

Interestingly, this may explain why many women improve after menopause.

Once estrogen levels become consistently low after menopause, the dramatic hormonal fluctuations disappear. While estrogen remains lower, the brain is no longer exposed to the repeated hormonal swings that trigger migraine.

Does hormone therapy help?

This is where the conversation becomes more nuanced.

Many women understandably wonder whether menopausal hormone therapy (MHT) will improve their migraine.

The answer depends on the individual.

Recent reviews suggest that during early perimenopause, when the ovaries are still producing unpredictable amounts of estrogen, adding external estrogen may occasionally worsen migraine if it increases hormonal variability.

On the other hand, later in the menopausal transition, carefully selected women may experience improvement when hormone therapy provides more stable estrogen levels.

This highlights why migraine management during menopause should never follow a one-size-fits-all approach.

Treatment decisions depend on:

  • whether migraine occurs with or without aura
  • cardiovascular risk factors
  • smoking status
  • blood pressure
  • timing within the menopausal transition
  • severity of menopausal symptoms
  • overall health goals

This is one reason why a comprehensive menopause assessment is so valuable before initiating treatment.

What this means for patients

If your migraine has changed during your 40s, you are not imagining it.

Many women tell me:

“I’ve never had headaches before.”

“My migraines suddenly became much worse.”

“My usual medication doesn’t work anymore.”

These experiences are increasingly supported by scientific evidence.

Hormonal fluctuations can influence both the frequency and severity of migraine, while sleep disruption, stress, anxiety, and vasomotor symptoms may further amplify attacks.

The encouraging news is that effective management often requires addressing all of these contributors rather than simply prescribing another pain medication.

Optimizing sleep, regular physical activity, nutrition, stress management, appropriate migraine-specific therapy, and individualized hormone management may all play important roles.

Limitations of the study

Like all research, this study has limitations.

It included only 66 women and measured hormone levels at a single point in time. Because estrogen fluctuates dramatically throughout perimenopause, a single blood test cannot fully capture the hormonal changes occurring over weeks and months.

Larger longitudinal studies with repeated hormone measurements will provide a clearer understanding of how changing estrogen patterns influence migraine.

Lifestyle Medicine Solutions:

While hormones play an important role, they are only one piece of the puzzle. Lifestyle Medicine offers several evidence-based strategies that can reduce migraine frequency and severity. Prioritizing consistent, restorative sleep, engaging in regular aerobic exercise and resistance training, eating balanced meals with adequate protein while avoiding prolonged fasting, maintaining good hydration, managing stress through mindfulness, cognitive behavioural therapy or relaxation techniques, and limiting alcohol are all supported by migraine research. Identifying individual triggers, such as sleep deprivation, skipped meals, dehydration or certain foods, can also be helpful. For women who are overweight or insulin resistant, improving metabolic health may further reduce migraine burden. These lifestyle interventions not only help manage migraine but also improve cardiovascular health, mood, sleep quality and overall wellbeing during the menopause transition.

Nutrition and Supplement Solutions:

Nutrition can also play an important role in migraine management, although responses vary from person to person. Eating regular, balanced meals helps prevent blood sugar fluctuations, a common migraine trigger, while a Mediterranean-style dietary pattern rich in vegetables, fruit, legumes, whole grains, nuts, seeds and omega-3-rich fish may help reduce systemic inflammation. Some women find benefit from identifying individual dietary triggers such as alcohol (particularly red wine), excessive caffeine or caffeine withdrawal, aged cheeses, processed meats containing nitrates, monosodium glutamate (MSG), or artificial sweeteners, although routine elimination diets are not recommended without a clear pattern. Staying well hydrated is equally important, as dehydration is a frequent trigger. Warm peppermint tea may provide symptomatic relief for some people by promoting relaxation and easing associated nausea.

Several nutritional supplements have the strongest evidence for migraine prevention. Magnesium (particularly magnesium glycinate or citrate, 300–600 mg daily) may reduce migraine frequency. Riboflavin (vitamin B2) at 400 mg daily has demonstrated benefit in reducing migraine attacks, while coenzyme Q10 (100–300 mg daily) has also shown preventive effects in clinical trials. Some evidence supports omega-3 fatty acids for reducing neuroinflammation.

One of the greatest frustrations women share with me is feeling dismissed when they develop new migraine during perimenopause.
